Your First Appointment
1. We are located at . The plaza is on
the south side of Bell Rd in surprise between West Point Parkway and Litchfield Rd.
You will see Chilli’s and the Coblestone Car Wash closest to Bell Rd.
2. Head South into the plaza, past Cobblestone and Chilli’s towards the Sprouts grocery store. Sprouts is 2 doors to the left of our office, and a Dry Cleaners is located to our Right when facing the storefront.
3. As you walk through our door you will be greeted by our friendly staff, asked to sign in and complete our New Patient Forms to help us learn more about you. To save time these forms are available on this site in our New Patient Center. We will also ask for any health insurance information you have so that we may verify your benefits and coverage for you.
4. Once your paperwork is completed a member of our staff will guide you back to our consultation room. Here we will ask you more specific questions about your health history and current condition.
5. Next, Dr. Sheitelman will review your information and history then visit with you for your consultation. After your consultation a staff member will report to you on any and all costs of treatment, including what is and is not covered by any insurance you may have.
6. After the consultation you will be taken to our X-Ray room where Dr. Sheitelman will take the images he needs to diagnose and treat your condition.
7. After your X-rays are completed Dr. Sheitelman will take time to review your images, diagnose your condition, and formulate a treatment plan for you. Once the Doctor has completed his treatment plan you will be brought back to the consultation room where he will report his findings and recommendations to you. Now you can decide whether or not to accept treatment. You are welcome to consult again with our staff regarding any cost and payment issues.
8. This concludes your first appointment. You may now schedule your first treatment. In most cases this can be done on the same day.
Your First Treatment
1. As you enter the office you will be greeted by staff, asked to sign in and then take a seat in our waiting area.
2. You will be handed your folder and called back to one of our treatment rooms where Dr. Sheitelman will perform Chiropractic adjustments to treat your condition.
3. After your adjustment you can chose to receive Manual Therapy, to be provided by one of our Licensed Massage Therapists. Simply place your folder to the back of the Manual Therapy waiting box. Most times there is no wait, and wait time is rarely more than 15 minutes.
4. After Manual Therapy you can take
your folder to the front desk to make payment and schedule future appointments.
